Cleveland Clinic Lutheran Hospital in Cleveland is seeking a Licensed Practical Nurse (LPN) for the 4D Medical Unit—Med/Surg and Telemetry. This night shift position offers patient care across cardiac, respiratory, and other services, with focus on ADLs, medications, and education within the Cleveland Clinic system.

You will collaborate with RNs to plan and execute care, document assessments, and support patients and families, while meeting BLS and Ohio licensing requirements.

Qualifications

• Graduate of an approved school of Practical Nursing.
• One year of clinical experience within the previous three years or must have graduated within the previous six months of hire.
• LPN licensure and current registration in the State of Ohio.
• For specific areas, must be certified in medication administration and IV therapy as approved by the Ohio Board of Nursing.
• Basic Life Support (BLS) through American Heart Association or American Red Cross within one year of hire.

Responsabilités

• Perform admission, ongoing assessments and discharges within defined time frames; assess and document patients’ pain levels using protocols.
• Collaborate with the RN on care plan formulation, implementation, evaluation and revision.
• Notify RN of pertinent physical, psychosocial, and emotional findings.
• Provide direct care and complete hourly rounding; document rounding log.
• Delegate and evaluate care by non-licensed personnel.
• Administer prescribed medications per nursing protocols and ensure medication safety.
• Assess patient education needs and provide teaching to patients/families.
