Responsibilities

• Provides direct patient care under the supervision of an RN, including monitoring vital signs and assisting with activities of daily living.
• Conducts focused patient assessments and reports changes in patient conditions.
• Administers medications, treatments, and injections as prescribed.
• Assists in development, implementation and evaluation of patient care plans.
• Maintains accurate documentation of patient care and observations.
• Educates patients and families on health maintenance and post-discharge care.
• Ensures adherence to infection control protocols and safety standards.
• Assists with basic procedures such as wound care and specimen collection.
• Participates in orientation and training of new staff.
• Collaborates with interdisciplinary teams to deliver coordinated care.
• Prepares and maintains equipment and supplies for patient care.
• Responds to medical emergencies and supports code team interventions.
• Telemetry LPN: record and report telemetry readings and monitor vital signs.
