Guardian center in Massachusetts is seeking a compassionate LPN for a 3rd shift role. You will conduct assessments, document changes, and manage admissions and discharges. Wound care and emergency response are part of the responsibilities, with you leading CNAs and CMTs to ensure compliance with facility protocols.

• Conduct assessments, follow protocol, report changes and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, chart setup, orders, and medications.
• Manage discharges: document, notify physician/guardian, arrange transportation.
• Provide wound care following protocols and dressings.
• Lead CNAs and CMTs, ensuring policy compliance.
• Respond effectively to emergencies.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
