Job summary

Banner Health Corporate is seeking an ED Case Manager for Banner University Medical Center Tucson's Emergency Department. This role coordinates comprehensive case management, discharge planning, and utilization management to ensure safe, timely patient transitions 24/7.

The ideal candidate will blend social work expertise with care coordination to navigate payer requirements, community resources, and post-acute placements for diverse patient populations in a Level 1 Trauma Center environment.

Qualifications

• RN with active license and case management or utilization review knowledge
• MSW with LMSW/LCSW or MSW with plan to licensure within 6 months
• Strong care coordination, discharge planning, and interprofessional collaboration skills
• Ability to navigate payer requirements and community resources

Responsibilities

• Coordinate care across the health care continuum for assigned patients
• Develop and manage interdisciplinary discharge plans
• Communicate effectively with physicians, payers, and care team to ensure timely transitions
• Educate care team on case management concepts and best practices
• May supervise junior staff and participate in process improvements
