Qualifications

• Masters degree in a human services field.
• One year of full-time post-graduate mental health experience with the population served.
• One year of full-time post-graduate supervised experience in alcoholism and drug abuse counseling.

Responsibilities

• Provide quality case management and support to inpatient social workers.
• Coordinate with local agencies to facilitate collateral information, treatment referrals, and ongoing communications for patient care.
• Support patient treatment and discharge planning and ensure coordination with clinical teams.
