Job summary

Willow Creek Behavioral Health in Green Bay, WI is seeking a Behavioral Health Associate I. This on-site role supports a multidisciplinary treatment team, monitors patient behaviors, and provides continuous patient care with supervision from a licensed nurse.

The position includes evening shifts 6:00pm-6:30am and rotates weekends/holidays. Requirements include a high school diploma, CPR and de-escalation/restraint certification, and at least six months of related experience.

Qualifications

• High school graduate or equivalent.
• Six months or more experience with the population served preferred.
• CPR and de-escalation and restraint certification required.

Responsibilities

• Ensure the wellbeing of patients and provide a safe, positive environment under direction of charge nurse or supervisor.
• Maintain constant supervision of patients.
• Assist with activities of daily living (toileting, bathing, dressing, grooming, meals, hydration).
• Conduct safety checks and ensure supervision at required intervals.
• Check patients for contraband and manage valuables.
• Interact with patients and report significant observations to supervisor.
• Engage patients in activities to support treatment goals.
• Facilitate groups on social skills, coping, and independent living skills.
• Take and record vital signs and report significant changes to the clinical team.
• Chart in patient records accurately and timely.
• Apply de-escalation techniques and assist with restraints when necessary.
• Assist with unit cleaning and discharge paperwork.
• Initiate patient/employee incident reports.
• Coordinate patient transfers as required.
• Receive shift report and provide thorough handoff to next shift.
