Job summary

Cincinnati Children’s is seeking an experienced Registered Nurse Care Manager to join our Neonatal Intensive Care Unit (NICU) team at the Burnet Campus. The RN Care Manager coordinates complex care, supports families during hospitalization and transition home, and collaborates with physicians, social work, therapy, case management, and other teams to remove barriers to care.

This on-site, Monday–Friday day-shift role is 1.0 FTE with occasional weekend coverage and rotating holiday call.

Qualifications

• BSN required and RN licensure in Ohio.
• Minimum 5 years of professional nursing experience.
• Strong clinical assessment, communication, organization, and care coordination skills.
• Ability to collaborate effectively with patients, families, and interdisciplinary teams.

Responsibilities

• Evaluate timeliness and availability of treatments and adjust the plan of care as needed.
• Monitor outcomes and manage transitions from hospital to home or next level of care.
• Coordinate discharge planning, home care, DME, and post-acute resources.
• Lead care team rounds and maintain patient- and family-centered communication.
