Job summary

Qualifications

Responsibilities

• Assess patient condition and coordinate nursing care with other health disciplines involved in the treatment effort.
• Develop a written individualized care plan including teaching needs and discharge planning.
• Administer direct care and medications as needed.
• Practice infection control and maintain a clean and safe environment.
• Communicate changes in patient condition to Charge RN/Clinician.
• Develop and maintain the written plan of care and set patient goals with patient/family.
