Job summary

Qualifications

Responsibilities

• Assess patient needs using the nursing process and plan and implement individualized interventions.
• Coordinate with health care team to ensure patient-centered care and family involvement.
• Promote patient safety and reduce risk through evidence-based practices.
• Coordinate discharge planning with interdisciplinary team.
• Lead care team and delegate appropriately.
• Maintain compliant documentation in EMRs.
