Qualifications

• Minimum 2 years Level III NICU experience.

Responsibilities

• Coordinate patient care for NICU patients across the care team.
• Facilitate transition from hospital to home or within the community.
• Educate families and provide referrals as indicated.
• Monitor quality indicators and participate in performance improvement initiatives.
