Resourceful Talent Group is seeking an experienced Service Coordinator to support NHTD Waiver participants in New York. You will independently manage a caseload, develop and adjust service plans, and collaborate with families and providers to ensure continuity of care.

Qualifications

• Hands-on NHTD Waiver experience is required.
• Previous Service Coordination experience is preferred.
• Knowledge of NHTD regulations, service plans, and care coordination.
• Strong communication, organizational, and case management skills.
• Ability to work independently and manage multiple cases.
• A reliable, professional, and compassionate approach.

Responsibilities

• Coordinate services and care for NHTD Waiver participants.
• Develop, review, and update service plans based on participants' needs.
• Communicate with participants, families, caregivers, and service providers to support continuity of care.
• Monitor service delivery, follow up on outstanding needs, and address coordination issues.
• Maintain accurate case records and documentation in accordance with program requirements.
• Manage a caseload independently while maintaining timely communication and follow-up.
