Job summary

Porter Cares is expanding into New Jersey and seeks Registered Nurses to perform in-home health risk assessments across the central region. You will evaluate medical history, conduct comprehensive exams, and coordinate care with Porter Care Guides to improve patient outcomes.

The role focuses on empathy, clear communication, and reliable documentation as part of a patient-centered healthcare ecosystem. Travel is required within Hunterdon, Mercer, Middlesex, Monmouth, and Somerset counties.

Qualifications

• Valid unencumbered New Jersey NP license.
• 3 years of patient care experience required (primary care/adult/geriatric; EMR skills preferred).
• Strong empathy, compassion and friendliness; excellent written and oral communication.

Responsibilities

• Visit members within your geographic area and conduct an in-home health risk assessment with medical history, exam, and medication reconciliation.
• Perform vital signs, clinical judgment, and care management planning to inform Porter Care Guides.

What You'll Do

You will visit members within your geographic area of responsibility and conduct an in-home health risk assessment including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam. This evidence-based practice screening will include point-of-care testing (as appropriate), the identification of diagnoses to be used in care management by our Porter Care Guides and active medical management in the furtherance of treatment, and the formulation of a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.

What You'll Need
