Job summary

Porter-Cares, Inc. is expanding into New Jersey and seeks Registered Nurses to join our in-home health program.

You will travel within Atlantic, Burlington, Camden, Cape May, Cumberland, Gloucester, Ocean and Salem counties to assess patients in their homes. Responsibilities include comprehensive health risk assessments, review of medical history, medication reconciliation, vital signs, and care planning with Porter Care Guides.

Qualifications

• Valid New Jersey RN license.
• 3 years of patient care experience required (primary care/adult/geriatric; EMR skills preferred).
• Strong empathy, compassion, and friendliness.
• Excellent written and oral communication with patients and coworkers.

Responsibilities

• Visit members within the geographic area and conduct in-home health risk assessments.
• Evaluate medical history, review systems, reconcile medications, and perform vital signs and physical exams.
• Identify diagnoses for care management and formulate care plans with Porter Care Guides.

What You'll Do

You will visit members within your geographic area of responsibility and conduct an in-home health risk assessment including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam. This evidence-based practice screening will include point-of-care testing (as appropriate), the identification of diagnoses to be used in care management by our Porter Care Guides and active medical management in the furtherance of treatment, and the formulation of a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.

What You'll Need

• Exceptional communication skills, both written and oral, with the ability to assist others (patients and coworkers alike) with sincerity, respect, and compassion

Diagnostic visits are paid per completed visit, averaging 35-minutes to 45-minutes per visit.
