Job summary

Cleveland Clinic Akron General Hospital in Akron, OH is seeking an experienced RN for night shift in the Transitional Care/Level II PCU. You will deliver direct nursing care, assess and plan care, and coordinate interdisciplinary plans of care.

Responsibilities include ongoing assessments, patient/family education, documentation, discharge planning, and participation in quality improvement. Minimum qualifications include ADN/BSN, RN license, BLS, ACLS, and ICU/acute care experience.

Qualifications

  • Graduate from an accredited School of Professional Nursing (ADN or BSN).
  • Current state RN licensure in good standing.
  • Basic Life Support (BLS) certification required.
  • ACLS certification required within one year of hire.
  • One year of ICU or acute care RN experience.

Responsibilities

  • Provide direct nursing care and monitor patient status.
  • Assess, plan, implement, and evaluate care for a patient population.
  • Complete ongoing assessments and respond to changes in condition.
  • Develop goals and involve patients and families in care plans.
  • Coordinate interdisciplinary plan of care and act as patient advocate.
  • Administer treatments and medications and document accordingly.
  • Participate in quality improvement and discharge planning discussions.
  • Delegate tasks and pursue ongoing education and licensure requirements.
  • Document progress and discharge goals with patients and families.
  • Communicate plan of care and acuity at shift report.

Akron General’s Transitional Care Unit is converting to a Level II Progressive Care Unit, where caregivers will be able to provide a new, higher level of intermediate care to our patients. The patient population in this unit will consist of non-invasive BIPAP, continuous pulse ox monitoring, insulin and cardiac/vasoactive GTTs, chronic trach ventilators, acute stroke patients, subarachnoid spinal drains, transvenous pacers and arterial waveform monitoring. As an RN, you will deliver exceptional care, throughout the patient care journey. In this role, you will interact directly with patients and their families, offering the opportunity to enhance their experience and provide healthcare education as needed.

A caregiver in this position works nights from 7:00PM – 7:30AM.
A caregiver who excels in this role will:
  • Provide direct nursing care.
  • Assess, plan, implement and evaluate care for a given patient population.
  • Complete ongoing assessments, recognize changes in patient condition and respond appropriately.
  • Develop and update goals and involve patients and families in developing plans of care.
  • Coordinate the interdisciplinary plan of care and serve as a patient advocate.
  • Implementation: provide nursing care, administer treatments and medication, and document.
  • Participate in ongoing interdisciplinary evaluation of patient/family outcomes, contribute to the improvement of care, and participate in the quality improvement process.
  • Delegate tasks/assignments, serve as a resource person, pursue ongoing education and specialty certifications, maintain applicable certification/licensure, and obtain any required certifications specific to the unit.
  • Review and document progress and discharge goals with patients and families.
  • Participate in and document patient care, discharge planning and case management conferences.
  • Communicate information regarding patient plan of care, current outcomes and acuity at shift report.
  • Communicate changes in patient condition and patient care/unit issues, ensure JCAHO Core Measures related to nursing care are achieved, incorporate patient safety goals and complete all paper and electronic records.
Minimum qualifications for the ideal future caregiver include:
  • Graduate from an accredited School of Professional Nursing (ADN or BSN program)
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)
  • Advanced Cardiac Life Support (ACLS) Certification through the American Heart Association (AHA) upon or within one year of hire
  • One year of current ICU or acute care RN experience in a hospital setting
Preferred qualifications for the ideal future caregiver include:
  • Bachelor of Science in Nursing (BSN)
Physical Requirements
  • Requires full range of body motion including handling and lifting patient, manual and finger dexterity, and eye-hand coordination.
  • Requires standing and walking for extended periods of time.
  • Requires corrected vision and hearing to normal range.
  • Requires working under stressful conditions and irregular hours.
  • Exposure to communicable diseases and/or body fluids.
  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical Demand requirements are in excess of those for Light Work.
Personal Protective Equipment
  • Follows standard precautions using personal protective equipment as required
