Job summary

Integrity Home Care + Hospice is seeking new graduate Physical Therapists to provide compassionate in-home care in Springfield, MO. You will receive mentorship, develop one-on-one patient relationships, and impact patients from day one.

We offer structured orientation, clinical support, and experienced PT mentors to ease transition from classroom to practice while ensuring high-quality care for adult and geriatric clients in their homes.

Qualifications

• Graduate of an accredited Doctor of Physical Therapy (DPT) program.
• Licensed or license-eligible Physical Therapist in the state of practice.
• New graduates are strongly encouraged to apply.
• Excellent communication and interpersonal skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and assess patients' functional status and mobility.
• Develop individualized treatment plans to maximize independence and safety.
• Provide skilled therapeutic interventions in patients' homes.
• Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
• Complete timely electronic documentation while maintaining the highest standards of patient care.
