Job summary

Fort Defiance Indian Hospital Board, Inc. seeks a licensed pharmacist to provide comprehensive pharmaceutical care across clinics and community sites within the Service Unit.

You will counsel patients, review therapy, prepare sterile and non-sterile formulations, and collaborate with physicians, nurses, and staff to ensure safe, effective medication use. Qualifications

• Completion of an accredited college or university with a Bachelor's degree in Pharmacy or Pharm.D.
• Active, unrestricted Pharmacy License in any U.S. State or Territory by completing necessary Board of Pharmacy requirement for the respective state of licensing.

Responsibilities

• Monitor, modify, and evaluate drug therapy; counsel patients on therapy and side effects.
• Compound sterile and non-sterile preparations; screen prescriptions for accuracy and interactions.
• Evaluate orders based on guidelines and patient factors; provide therapy recommendations.
• Dispense medications with accuracy and safety; monitor outcomes and toxicity.
• Participate in reconciliation, rounds, and collaborative treatment planning with clinicians.
• Provide pharmaceutical care across homes, schools, clinics, and community sites within the Service Unit.
• Oversee technicians; ensure dispensing accuracy, storage, and inventory compliance.
• Assist in policy development, training, and quality improvement initiatives.

Education

Bachelor's degree in Pharmacy or Pharm.D.

Active, unrestricted Pharmacy License in any U.S. State or Territory
