Job summary

Premier Nursing and Rehab Center of Far Rockaway is seeking an LPN to provide direct nursing care under RN supervision in a skilled nursing facility in New York. You will administer medications, monitor residents, and document care to support physical and emotional well-being.

The role requires NY LPN licensure, CPR/BLS, and strong communication in a fast-paced environment. Applicants should have a practical nursing program background and EMR familiarity.

Qualifications

• Current New York State LPN license.
• Graduate of an accredited Practical Nursing Program.
• Current CPR/BLS Certification.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively in a fast-paced skilled nursing environment.

Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans.
• Administer prescribed medications, injections, treatments, and wound care accurately and safely.
• Monitor residents for changes in condition and promptly report findings to the RN or physician.
• Complete accurate and timely documentation in the EMR.
• Assist with admissions, transfers, and discharges.
• Supervise CNAs and delegate resident care assignments appropriately.
• Ensure resident safety and maintain infection prevention and control standards.
• Respond promptly to resident needs, emergencies, and changes in medical condition.
• Communicate effectively with residents, families, physicians, and the interdisciplinary care team.
• Participate in care planning meetings and quality improvement initiatives.
• Maintain compliance with New York State Department of Health regulations, CMS guidelines, and facility policies.
