Job summary

Interim HealthCare Inc. is seeking a Licensed Practical Nurse (LPN) to join our home-based care team in Cincinnati, OH. You will deliver compassionate, in-home nursing services, ensuring comfort and dignity for clients while following individualized care plans.

The role involves 1:1 patient care, coordination with a cross-disciplinary team, and adherence to safety protocols. Flexible scheduling options are available with full-time or part-time opportunities and daily pay options.

Qualifications

• Graduate of a practical nursing program and valid LPN/LVN license in the state where you practiced.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites and/or work locations.
• Trach and/or ventilator experience preferred, not required.

Responsibilities

• Provide in-home direct patient care according to the plan of care and nursing scope.
• Assess and document patient conditions and report changes as needed.
• Administer medications, insulin, and IV fluids with thorough documentation.
• Inspect and care for wounds, change dressings and assist with personal hygiene.
• Assess for further injuries or infections and deliver appropriate treatments.
• Collaborate with providers and health professionals to ensure patient outcomes.

As a Licensed Practical Nurse (LPN) you will:

• Providein-homedirect patient care according to theprovider’sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery.
• Assessand document patient conditions and reportchangesas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV fluids:documenting thoroughly.
• Inspect and care for wounds, changing dressings and assisting with personal hygiene.
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.

To qualify for a Licensed Practical Nurse (LPN) position with us:

• Education: Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state(s) in which candidate completed at least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trach and/or ventilator experience preferred, not required
