Job summary

Interim HealthCare Home Care is seeking a Licensed Practical Nurse (LPN) to provide in-home direct patient care across a range of services, ensuring comfort and dignity for clients. The role offers truly flexible scheduling (FT, PT, PRN), competitive pay, benefits, incentives, and a Daily Pay option.

No overtime is required and care is delivered with 1:1 patient focus. Qualifications include a current unrestricted LPN license in the state of practice, active CPR/BLS/First Aid certification,

Qualifications

• Licensure: Current unrestricted LPN license in the state of this position
• CPR/AED/BLS/First Aid certification current
• Reliable transportation to/from care sites and/or work locations
• Trach/ventilator experience preferred, not required

Responsibilities

• Provide in-home direct patient care according to the plan of care and nursing scope of practice.
• Assess and document patient conditions; monitor vitals and report changes.
• Administer medications, insulin, and IV fluids; document thoroughly.
• Inspect and care for wounds, change dressings, assist with personal hygiene.
• Assess for further injuries or infections and deliver appropriate treatments.
• Collaborate with providers, aides, and other health professionals to ensure patient outcomes.

As a Licensed Practical Nurse (LPN) you will

• Providein-homedirect patient care according to theprovider’sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery.
• Assessand document patient conditions and reportchangeas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV fluids:documenting thoroughly.

• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.

To qualify for a Licensed Practical Nurse (LPN) position with us

• Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trach and/or ventilator experience preferred, not required
