Job summary

Interim HealthCare Inc. is seeking a Licensed Practical Nurse (LPN) for in-home patient care in Mann's Choice (Bedford area). Flexible schedules include full-time, part-time, and PRN options.

You will provide direct patient care, monitor progress, administer medications, and collaborate with the health care team to deliver compassionate services. Ideal candidates demonstrate a nursing degree, an unrestricted state LPN license, CPR/BLS/First Aid certification, and reliable transportation.

Qualifications

• Graduate of an accredited nursing program.
• Current unrestricted LPN license in the state.
• CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites.
• Trach/vent experience preferred, not required.

Responsibilities

• Provide in-home direct patient care according to the prescribed plan of care and nursing scope.
• Assess and document patient conditions and report changes as needed (vital signs, fluids, etc.).
• Administer medications, insulin, and IV fluids with thorough documentation.
• Inspect and care for wounds and assist with personal hygiene.
• Assess for injuries, infections or bedsores and deliver appropriate treatments.
• Collaborate with providers, aides and allied health professionals to ensure patient outcomes.

Schedules: 5AM - 3PM or 10PM - 10AM

• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trachand/or ventilator experience preferred, not required
