Job summary

Community Care Physicians is seeking a Nurse Practitioner or Physician Assistant to join CapitalCare Neurology in Niskayuna, NY. You will work with an established neurology team to evaluate and manage patients in an outpatient setting, developing expertise in neurology and coordinating care with other providers.

The role offers a starting salary of $125,000+ with a 4-day work week option, comprehensive benefits, and on-site EMG/EEG, EEG, and ambulatory EEG capabilities.

Qualifications

Current NYS license as PA or NP and board certification.
Experience in ambulatory/outpatient neurology preferred.
Strong history and neurologic examination skills.
Ability to order and interpret labs/imaging and develop treatment plans.

Responsibilities

Evaluate, diagnose, and manage patients with neurological conditions.
Perform comprehensive histories and neurological examinations.
Develop and implement individualized treatment plans.
Coordinate care with PCPs and specialists.
Document thoroughly in the EMR.
Provide patient education and medication management.
Maintain longitudinal follow-up and ongoing management.
Support on-site EMG/EEG testing.
