Job summary

Community Care Physicians is seeking a Nurse Practitioner or Physician Assistant to join CapitalCare Neurology in Niskayuna, NY. This is an excellent opportunity to work in an established outpatient neurology practice alongside experienced neurologists and APPs, developing expertise in this specialty.

The role offers a starting salary of $125,000+ with a 4‑day work week option, and on-site EMG/nerve conduction studies, EEG, and ambulatory EEG.

Qualifications

• Licensed and active registration as a physician assistant or nurse practitioner in New York State.
• PA: Bachelor's degree, NYS-accepted PA program, NCCPA certification, ambulatory PA experience, CPR.
• NP: NP education program, certification by AANPCB or ANCC, pharmacology coursework, ambulatory nursing experience, CPR.

Responsibilities

• Evaluate, diagnose, and manage patients with neurological conditions.
• Perform comprehensive histories and neurological examinations.
• Develop and implement individualized treatment plans.
• Manage acute and chronic neurological conditions.
• Order and review labs, imaging, and diagnostics.
• Provide patient education and medication management.
• Coordinate care with primary care providers and specialists.
• Provide longitudinal follow‑up and ongoing management.
• Maintain accurate EMR documentation.
