Job summary

Community Care Physicians is seeking a Nurse Practitioner or Physician Assistant to join CapitalCare Neurology in Niskayuna, NY. You will collaborate with experienced neurologists and APPs to develop expertise in outpatient neurology and provide comprehensive neurological care.

The role emphasizes evaluation, diagnosis, and management of diverse neurological conditions within a well-established practice, with on-site EMG/nerve studies, EEG, and ambulatory EEG.

Qualifications

• Active NYS license and registration as PA or NP.
• Graduated from an accredited PA studies program or accredited NP program.
• Experience in ambulatory health care preferred.
• Strong teamwork with neurologists and APPs.

Responsibilities

• Evaluate, diagnose, and manage patients with neurological conditions.
• Perform comprehensive histories and neurological examinations.
• Develop and implement individualized treatment plans.
• Manage acute and chronic neurological conditions.
• Order and review laboratory, imaging, and diagnostic testing.
• Provide patient education and medication management.
• Coordinate care with primary care providers and specialists.
• Maintain accurate documentation in the EMR.
