Job summary

Infinite Medical P.C. is seeking a friendly and compassionate Neurology Nurse Practitioner or Physician Assistant to join our team in Montgomery, NY. This part-time role focuses on delivering comprehensive neurological care to residents in long-term care facilities.

You will develop treatment plans, perform assessments, and collaborate with a multidisciplinary team to provide high-quality patient care while ensuring documentation and education for patients and families.

Qualifications

• Minimum 6 months of experience in long-term care or skilled nursing facility
• Active and unrestricted NP, PA, or MD license
• DEA License
• CPR BCLS/BLS certified
• Must hold a Master's Degree in Nursing
• Completed a degree from an accredited NP program
• Neurology experience preferred
• EMR documentation experience (Sigma, Visual, PCC, etc.) training provided

Responsibilities

• Develop and implement individualized treatment plans for patients, including medication management and non-pharmacologic interventions
• Conduct regular follow-up visits to monitor patient progress and adjust treatment plans
• Collaborate with physicians and other healthcare professionals to coordinate care
• Provide patient and family education on disease management, medications, and lifestyle modifications
• Maintain accurate and timely documentation of patient encounters and treatment plans
• Participate in quality improvement initiatives to enhance patient outcomes
• Perform comprehensive assessments to diagnose and treat neurological conditions

Job Summary

• Perform comprehensive assessments to accurately diagnose and treat patients with neurological conditions

Requirements

• Minimum 6 months of experience in long-term care or a skilled nursing facility


• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license


• DEA License


• CPR BCLS/BLS certified


• Must hold a Master's Degree in Nursing


• Completed a degree from an accredited Nurse Practitioner program


• A year of experience in Neurology


• Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided


• Must have great organizational and time management skills.


• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.
