Infinite Medical P.C. is seeking a Neurology Nurse Practitioner/Physician Assistant for a part-time role in Hamden and Danbury.

You will provide compassionate care to residents with neurological disorders, develop treatment plans, and work with a multidisciplinary team to improve patient outcomes in skilled nursing facilities. The position requires NP/PA licensure, a Master’s in Nursing, 1 year in neurology preferred, and DEA/CPR/BLS certification.

Qualifications

• Minimum 6 months of experience in long-term care or skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA License required.
• CPR BCLS/BLS certified.
• Master's Degree in Nursing.
• Completed a degree from an accredited Nurse Practitioner program.
• One year of neurology experience preferred.
• Experience with EMR documentation training provided.

Responsibilities

• Develop and implement individualized treatment plans for patients, including medication management and non-pharmacologic interventions.
• Conduct regular follow-up visits to monitor progress and adjust treatment plans as necessary.
• Collaborate with physicians and other healthcare professionals to coordinate care.
• Provide patient and family education on disease management, medications, and lifestyle modifications.
• Maintain accurate and timely documentation of patient encounters and treatment plans.
• Participate in quality improvement initiatives to enhance patient outcomes and care delivery.
• Perform comprehensive assessments to diagnose and treat neurological conditions.

Job Summary

Requirements

• Must hold a Master's Degree in Nursing
• Completed a degree from an accredited Nurse Practitioner program
• A year of experience in Neurology
• Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc): training provided
• Must have great organizational and time management skills.
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.
