Elaya Health in New York City is seeking a compassionate Nurse Practitioner with a neurology background to provide high-quality care to residents in long-term care facilities. You will perform neurological evaluations, diagnose and manage conditions, and collaborate with nursing home staff and families.

Qualifications
- Licensed NP in New York State.
- Board Certification in Neurology or experience in neurology preferred.
- Minimum 1-2 years NP experience (geriatrics or neurology a plus).
- Experience in long-term care or nursing home settings preferred.
- Strong clinical judgment and the ability to work independently.
- Excellent communication and documentation skills.
Responsibilities
- Provide neurological evaluations and follow-up care for nursing home residents.
- Diagnose and manage conditions such as stroke recovery, Parkinson's disease, dementia, neuropathy, and other neurological disorders.
- Collaborate with nursing home staff, families, and interdisciplinary teams to create comprehensive care plans.
- Document all patient encounters and maintain compliance with healthcare regulations.
- Educate patients, caregivers, and facility staff on neurological conditions and treatments.
