Job summary

Infinite Medical P.C. is seeking a Neurology Nurse Practitioner or Physician Assistant to join our team in Queens, NY. You will deliver comprehensive care to residents with neurological disorders in a long‑term care setting.

The role emphasizes medication management, patient education, and collaboration with physicians to coordinate care. A Master’s in Nursing and an accredited NP program are required, along with an active NP/PA license and DEA.

Qualifications

• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• DEA License.
• CPR BCLS/BLS certified.
• Must hold a Master's Degree in Nursing.
• Completed a degree from an accredited Nurse Practitioner program.
• A year of experience in Neurology.

Responsibilities

• Develop and implement individualized treatment plans for patients, including medication management and non-pharmacologic interventions.
• Conduct regular follow-up visits to monitor patient progress, adjust treatment plans as necessary, and provide education and support.
• Collaborate with physicians and other healthcare professionals to coordinate care and ensure a multidisciplinary approach.
• Provide patient and family education on disease management, medications, and lifestyle modifications.
• Maintain accurate and timely documentation of patient encounters and treatment plans.
• Participate in quality improvement initiatives to enhance patient outcomes and optimize patient care delivery.
• Perform comprehensive assessments to accurately diagnose and treat patients with neurological conditions.
