Job summary

Rehab Without Walls Neuro Rehabilitation in Pasadena, CA seeks a licensed Physical Therapist to deliver functional rehabilitation in home and community settings. Responsibilities include evaluations, individualized plans, and progress communication with families and care teams.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support or CPR certification.
• Minimum of one year of work experience as a physical therapist preferred.
• Knowledge of rehabilitation techniques for complex neurological injury.
• Ability to lift 50 pounds; can lift up to 100 pounds following safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess treatment effectiveness; attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations and resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to ensure coordinated care and outcomes.
