The Upstate Neurological Institute Registered Nurse Navigator (UNI RN Navigator) functions as a core member of an interdisciplinary care team, offering expert clinical guidance, care coordination, and patient advocacy for neurological conditions throughout the continuum of care.

Emphasis is on stroke, neurotrauma, neurosurgical care, and recovery, coordinating access to diagnostics, therapies, rehab, and support services.

Qualifikationen

• NYS licensed/registered RN or eligible.
• Bachelor's degree in Nursing or healthcare field.
• 3 years post-licensure RN experience (preferred in specialty) or 5 years with an Associate degree.
• Excellent written and verbal communication and interpersonal skills.
• CPR-BLS certification required by time of appointment.

Aufgaben

• Provide expert clinical guidance, care coordination and patient advocacy across the continuum of care.
• Screen for psychosocial concerns and facilitate appropriate referrals.
• Serve as a liaison among patients, families, providers and teams to promote continuity of care.
• Support standardized processes to enhance care coordination, access and outcomes.
