Job summary

BrightSpring Health Services through Rehab Without Walls Neuro Rehabilitation seeks a Physical Therapist / Case Manager to deliver 1:1 community-based physical therapy while coordinating patient care.

You will collaborate with an interdisciplinary team to build individualized rehab plans for adults recovering from brain injury, stroke, spinal cord injury, and other neurological conditions, in home and community settings.

Qualifications

• Bachelor's degree in Allied Health or related field.
• Current licensure/registration in the state of practice.
• CBIS certification preferred.
• BLS/First Aid and CPR certifications required.
• Minimum 2 years rehabilitation experience with neurologically impaired patients.
• Prior case management or insurance administration experience preferred.
• Strong organization, problem solving, and communication skills.

Responsibilities

• Manages individual patient services and collaborates with clinical staff to develop rehabilitation plans.
• Formulates transdisciplinary rehabilitation plans, outcomes, and goals with patients and the team.
• Completes evaluation reports and monitors patient status with regular updates.
• Monitors satisfaction of patient, family, physician, payer, and case manager.
• Leads interdisciplinary case conferences and communicates progress to physicians and families.
• Schedules case conferences with clinical team, payers, physicians and family members.
• Communicates progress with patient and care partners and coordinates equipment acquisition.
• Participates in medical provider meetings to address issues for the patient.
• Monitors hours of service and reports changes to leadership.
• Negotiates extensions of service with funding sources.
• Obtains and tracks orders from physicians for ongoing services.
• Adheres to Code of Conduct, HIPAA, and applicable laws.
• Attends internal staff development and maintains clinical skills.

Education

Bachelor's degree in Allied Health (OT, PT SLP), Registered Nurse, Human Services, or Rehabilitation Counseling/Psychology

Current BLS First Aid and CPR certifications
