Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a qualified nutrition professional to provide patient-centered dietary services in Denver, CO. You will assess nutritional needs, design therapeutic diets, and collaborate with physicians to optimize treatment plans within a home and community-based care model.

The role requires a Nutrition degree and active Registered Dietitian licensure, plus at least six months of related experience.

Qualifications

• Degree in Nutrition required, or the equivalent.
• Current Registered Dietician licensure/certification in the state(s) of practice.
• Minimum of six months related experience generally required.

Responsibilities

• Provides professional nutritional services in compliance with state practice acts and healthcare standards.
• Interviews patients and assesses/evaluates their nutritional status to determine dietary needs.
• Consults with physicians to establish dietary treatments and prepares therapeutic diets.
• Explains dietary modifications to patients and families and educates on restrictions and interactions.
• Monitors patient response to dietary treatment and revises plans as needed.
• Maintains confidentiality of patient files and documentation.
• Maintains clinical practice skills through continuing education and staff development.
• Participates in quality improvement activities and data collection for root-cause analysis.
• Adheres to HIPAA and company privacy and ethics practices.
• Performs other duties as assigned.

Education
