Job summary

Albany Med Health System seeks a Patient Care Assistant (PCA) for the Neuro Progressive Care unit on the night shift. Under RN supervision, you will collect patient data, perform personal care, conduct point-of-care testing, assist with mobilization, and support discharge planning during acute care transitions.

Responsibilities include documenting care, reporting abnormalities, participating in care planning, and maintaining a safe environment.

Qualifications

• High School Diploma/G.E.D. or equivalent - required.
• Less than 1 year experience in a health care setting or completion of certified nurse assistant course - preferred.
• Experience as a paramedic, emergency technician - preferred.
• Basic knowledge of medical terminology (Medium proficiency).
• Honest, punctual, and performs the job in adherence to Albany Medical Center and the tenets of the profession.

Responsibilities

• Collects patient data including vital signs, height/weight, intake/output.
• Reports abnormal findings to the RN and care team.
• Participates in care planning and nursing reports.
• Maintains patient/family rights in decision making about care.
• Keeps patients and families informed using standardized communications.
• Provides personal patient care including bathing, oral and skin care.
• Assists with eating and hydration, grooming, dressing, and toileting.
• Promotes safe patient handling and restorative care such as ambulation and ROM.
• Maintains clean, orderly, and safe environment; follows infection control practices.
• Documents all care in the patient record.
