Job summary

Qualifications

• Completion of an accredited PT program with a PT license in California.
• National Provider Identifier (NPI) required at hire.
• Basic Life Support (BLS) certification required at hire.

Responsibilities

• Evaluate patients and determine dysfunction level and plan of care.
• Administer modalities and exercise programs.
• Document PT services per state and federal requirements.
• Coordinate treatment and discharge planning with care teams.
• Supervise PT assistants and aides when delegated.
