Job summary

Kaiser Permanente is seeking a Per Diem Outpatient Physical Therapist in San Francisco, CA. You will manage the physical therapy of patients under the direction of the Rehabilitation Services Director, providing direct treatment and coordinating with the care team.

The role requires a California PT license, a degree in physical therapy, and valid Basic Life Support. You will discharge patients when goals are met and document care accurately.

Qualifications

• Graduate of an accredited physical therapy curriculum with a bachelors, masters, or doctoral degree and licensure by date of hire.
• License: California Physical Therapist License.
• National Provider Identifier is required.
• Basic Life Support (BLS) required at hire.
• Must be able to work in a Labor/Management Partnership environment.

Responsibilities

• Evaluates patients and interprets evaluation findings to determine dysfunction and set goals.
• Initiates discharge planning and coordinates treatment with the care team.
• Discharges patients when maximum benefit is reached and documents care accurately.
• Provides education to patients and families and monitors progress to adjust goals.
• Collaborates with other health care personnel to ensure coordinated care.
