Job summary

Reid Health is seeking an Outpatient Neuro Physical Therapist to evaluate patients, develop and supervise therapy plans, and coordinate care with a multidisciplinary team. The role includes supervising PTAs, documenting progress, and engaging in discharge planning to ensure smooth transitions for patients.

Licensed Indiana PTs with BLS certification, the ability to work a day shift, and a commitment to Excellence, Empathy, Integrity, and Accountability are encouraged to apply.

Qualifications

• Must hold a degree in Physical Therapy (BS, MS, or DPT) from an ACTA-accredited program.
• Fully licensed in Indiana to practice as a Physical Therapist; OH license if providing services in Ohio.
• Basic Life support certification required.

Responsibilities

• Evaluate each patient using diagnostic, prognostic, and functional ability assessments.
• Re-evaluate patient status and modify care plan as needed.
• Supervise PTAs and rehab technicians; countersign PTA documentation.
• Document goals in functional, measurable terms with timeframes.
• Collaborate with patient, family, and other health professionals for discharge planning.
• Provide education to patients and caregivers regarding evaluation, treatment, and home programs.
• Participate in QA&I activities and promote PT services within the department.
