Advocate Health is hiring for an Advanced Practice Clinician in Wisconsin to support the inpatient stroke program within the Neuro Endovascular – Neurology department. The role covers acute and chronic patient care in an inpatient hospital setting, including history, exam, diagnosis, prescriptions, and care coordination.

The position requires PA/NP credentials with acute care experience, a nursing or PA background, and active licensure in the state.

Qualifications

• Physician Assistant license issued by the state in which the caregiver practices.
• PA-C certification by NCCPA.
• Registered Nurse license with NP certifications recognized by ANCC/AANP.

Responsibilities

• Evaluates, diagnoses, treats, and manages acute, critical and chronic conditions in an inpatient setting.
• Obtains histories, performs exams, prescribes therapies and orders diagnostics.
• Provides care coordination, discharge planning and referrals.
• Counsels patients on health, nutrition, and preventive care.
• Guides nursing staff and ensures adherence to care plans.
