Qualifications

Responsibilities
- Evaluate, diagnose, manage, and educate patients with kidney-related conditions.
- Provide care to patients in office, dialysis, and hospital settings.
- Collaborate closely with nephrologists, nursing teams, and other healthcare providers.
- Develop meaningful, ongoing relationships with patients.
- Complete timely documentation using Epic.
