Job summary

Qualifications

• Minimum 3 years of experience as an APP preferred.
• Experience in nephrology or chronic disease management strongly preferred.

Responsibilities

• Provide direct patient care, including assessment, diagnosis, treatment planning, and follow-up.
• Evaluate new and established patients in coordination with supervising physicians.
• Perform physical exams and assist with diagnostic or therapeutic procedures.
• Document patient interactions, histories, and care plans accurately in the electronic medical record (EMR).
• Collaborate with physicians and clinical staff to ensure high-quality care delivery.
