Formación

• Specialized Diploma Graduate of an accredited school of nursing.
• Knowledge of patient- and family-centered practice and collaboration.
• Effective communication with patients, families and care partners.
• Discharge planning and care coordination knowledge.
• Health information documentation proficiency.

Responsabilidades

• Interacts with co-workers, patients, their family/designated care partner(s), and other staff consistent with the values of Jefferson Health.
• Partners with patients and families in identifying learning needs and provides education.
• Uses information technology to communicate, manage knowledge, mitigate error, and support ethical decision making.
• Functions in nursing and inter-professional teams to achieve quality patient care.
• Creates an environment of belonging, supports learning and acts as a primary preceptor when appropriate.
• Participates in hospital- and practice setting committees focusing on safety and quality improvement.
