Job summary

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency serving Arizona. The Navigator II supports engagement, case management, and connections to needed services for individuals in crisis or recovery, coordinating with a multi-disciplinary team.

The Navigator II documents interactions per state and accrediting standards, and works to reduce barriers, guiding clients toward stable, ongoing access to resources and supports.

Qualifications

  • Highschool diploma or GED is required.
  • Associate degree in Behavioral Health is preferred but not required.
  • Minimum of six months of recovery from substance use and/or mental health disorders required.
  • 1–3 years of health care related work experience preferred but not required.
  • 1–3 years of substance abuse/mental health recovery-oriented service preferred but not required.
  • Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required.
  • Behavioral Health Technician (BHT) per program policies.
  • Current AZ Driver's License (valid with 39-month MVR).
  • Arizona Fingerprint Clearance Card required.

Responsibilities

  • Enhance likelihood of client success and reduce barriers to services.
  • Engage and map out next steps for access to services for ongoing success.
  • Conduct outreach, engagement, and wraparound supportive services.
  • Document and track required patient interactions per AAC/ADHS-DBHS/CARF policies and internal procedures.

Job Summary

The Navigator II’s primary role is to enhance the likelihood of success, identify and reduce barriers for clients and connect them with needed services and resources. The Navigator II will work with individuals who have been determined as in need of additional support and engagement and will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services. The Navigator II will also map out the most appropriate next steps for the individual who may need assistance in accessing various services to achieve ongoing success. The Navigator II is part of a multi-disciplinary team made up of internal and external stakeholders. The Navigator II will document, and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and specific for the program.

Skills/Requirements
  • Highschool diploma or GED is required. Associate degree (or higher) in a field related to Behavioral Health is preferred but not required of this position.
  • Minimum of six months of recovery from substance use and/or mental health disorders required.
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
  • 1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
  • Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
  • Current AZ Driver's License (valid and in good standing).
    Must be able to provide a 39-month MVR.
  • Arizona Fingerprint Clearance card. (Program Specific).
