Job summary

Open Sky Community Services seeks a Community Services Navigator to provide case management and outreach within our Mass Health funded program. You will coordinate care plans, connect members to community resources, and support transportation as they transition to community-based options.

The role requires strong communication, documentation, and organizational skills, with a commitment to serving high-risk individuals and coordinating with multiple providers to ensure integrated care.

Qualifications

• High School Diploma, GED or equivalent.
• Valid Driver’s License with reliable, insured transportation.
• Minimum 2 years’ experience or lived experience with homelessness, justice involvement, adults with serious and persistent mental illness, substance use disorders, and underserved populations.
• Excellent communication, documentation, time management and organizational skills.

Responsibilities

• Develop a Support Plan with the member that guides services in a way that is consistent and supportive of other existing comprehensive care plans. Complete all Assessments and Support Plans according to prescribed timelines and specifications.
• Act as a liaison between members, community agencies, and families to coordinate treatment, including employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others. Work collaboratively with medical and behavioral health providers to obtain any active comprehensive care plans that the member may have.
• Provide temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources.
• Participate in an On-Call rotation.

Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting. The Community Services Navigator will provide case management and direct outreach support to their assigned caseloads, which consist of members enrolled in Community Services Programs, Recovery Support Navigation, Behavioral Health Justice Involved and/or Community Support Program for People Experiencing Chronic Homelessness.

Pay Range

$22.07/Hr. - $24.81/Hr.
