Job summary

Affinity Health Management is seeking a Board Certified Music Therapist (or eligible to take the certification exam) to join our hospice team. The role involves delivering music therapy services, assessing patient needs, and guiding families and staff within an interdisciplinary team.

The position requires state certification, a relevant degree, CPR certification, and at least two years of experience, with preferred hospice exposure. A valid driver’s license is required for patient visits.

Qualifications

• Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
• Certified to practice as a music therapist in the state.
• Minimum of 2 years of music therapy experience; hospice experience preferred.
• Current CPR certification maintained.
• Understands hospice philosophy and focus on comfort over rehab.
• Licensed driver with a valid, insured vehicle.

Responsibilities

• Provide music therapy services per written care plan and assess progress.
• Direct music therapy treatment and evaluate patient needs.
• Determine priority music therapy goals for each patient.
• Report patient reactions and condition changes to the physician.
• Initiate therapy programs and train staff/families on goals.
• Prepare clinical summaries and progress reports.
• Provide music therapy consultation to families and caregivers.
• Participate in hospice team orientation and quality improvement activities.

Job Title/Position: Music Therapist

Reports To: Clinical Manager

JOB DESCRIPTION SUMMARY

The Music Therapist, contracted or employed, provides music therapy to address physical, emotional cognitive and social needs of patients.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

• 1. Provides music therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
• a. Assessing and evaluating therapeutic status, and participation in the development of the total plan of care. Evaluates home environment.
• b. Directing music therapy treatment.
• c. Determining priority needs for music therapy
• d. Reporting to physician patient's reaction to treatment or changes in condition.
• 2. Initiates music therapy program and instructs other personnel and/or family/caregivers in certain phases of music therapy with which they may work with a patient, as well as instructing them as to the goals of the music therapy program for the patient by participating in case conferences.
• 3. Prepares and submit clinical and progress summaries based on the attainment of goals.
• 4. Provides music therapy consultation to families/caregivers, as appropriate.
• 5. Provides in-service education programs for nursing peron [sic] as needed..
• 6. Participates in the interdisciplinary group.
• 7. Actively participates in the quality assessment performance improvement teams and activities.
• 8. Participates in an orientation program for hospice team members.

POSITION QUALIFICATIONS

• 1. Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
• 2. Certified to practice as a music therapist within the state.
• 3. At least (2) two years of appropriate experience as a music therapist. Community/Hospice experience is preferred.
• 4. Possesses and maintains current CPR certification.
• 5. Understands hospice philosophy, focus of care is comfort vs rehab
• 6. Licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
