Qualifications

Responsibilities

• Promotes patient- and family-centered care and educates on home care and medications.
• Participates in patient safety, quality, and regulatory compliance; pursues professional development.
• Develops, implements, and documents individualized care plans with the interprofessional team and patient/family.
• Proactively plans and communicates the plan of care across the continuum of care.
• Uses comprehensive nursing skills to assess physical, psychosocial, and emotional needs with attention to patient values and cultural considerations.
