Job summary

Piedmont Healthcare, Inc. is seeking a qualified Social Worker to promote patient and family wellness and support transitions of care in inpatient and Emergency Department environments. This role requires a Master’s Degree in Social Work and at least two years of acute hospital experience.

Qualifications

• Master’s Degree in Social Work is required.
• 2 years of experience in an acute hospital setting required.
• Licenses/certifications (LMSW, LCSW, ACM, CCM) required upon hire or as stated in the posting.

Responsibilities

• Promote patient and family wellness and improve care outcomes.
• Manage and develop transitional care plans for patients with complex and psychosocial needs in inpatient and ED settings.
• Provide Social Work Practice coaching to assigned staff to develop responsibility, skill, knowledge and ethics in practice.
