Job summary

Providence Milwaukie Hospital seeks an MSW Social Worker to provide biopsychosocial assessments and clinical interventions on the on-call day shift. You will collaborate with the care team to develop patient-centered plans and advocate for patients and families.

Under supervision, you will work across the care continuum to support outcomes, reduce barriers to care, and uphold NASW ethics. Oregon CSW Associate licensure within 60 days is required; hospice/benefit experience is preferred.

Qualifications

• Master's Degree in Social Work.
• Within 60 days of hire: Oregon Clinical Social Work Associate License.

Responsibilities

• Conduct comprehensive biopsychosocial/spiritual assessments, diagnoses and treatment/interventions/advocacy for patients and their support systems.
• Collaborate with patients and support systems to develop and implement person-centered care plans across the care continuum with the IDT.
• Provide care that respects patient self-determination and reduces health disparities while adhering to NASW ethics and SOP.
