Join us for the MSW Social Worker Maternal Infant Health role at Corewell Health.
Plans and prioritizes care for individuals and populations, focusing on strategies that promote optimal health. Provides peer support and role modeling related to life skills and health/wellness during home visits, if applicable. Demonstrates expertise in patient care and manages a caseload of clients with varying complexity, aiming to improve outcomes through educational concepts, skills, and preventive care in a managed care environment.
Required: Associate's Degree in nursing, social work, occupational or physical therapy, or related field; 2 years relevant experience; licensure in Michigan as RN, Physical Therapist, Occupational Therapist, Speech Language Pathologist, Physical Therapist Assistant, Occupational Therapy Assistant, Dietitian, MSW, Counselor, LPN; BLS certification; valid Michigan driver's license.
Preferred: Bachelor's Degree in related field.
