Job Summary

Plans and prioritizes care for individuals and populations of clients, focusing on strategies that promote optimal health within populations. Provides peer support and role modeling related to the development of life skills and health/wellness to individuals and families during home visits, if applicable. Demonstrates expertise and current knowledge in patient care and management of a caseload of clients of varying complexity, aiming to improve patient, family, and community health outcomes through educational concepts, skills, and preventive care in a managed care environment.

Essential Functions
  1. Visit homes to assess client and family needs, including performing health assessments such as blood pressure, pulse, BMI, and foot checks. Develop prioritized care plans and follow-up with families, community agencies, and volunteers to evaluate service effectiveness and plan future needs.
  2. Assess internal and external referrals to identify patient needs, insurance benefits, and resources.
  3. Develop care plans and collaborate with healthcare team members, including PCPs and specialists, to optimize care continuity.
  4. Assess educational needs of clients, families, and healthcare team members; develop and implement teaching strategies or referrals.
  5. Coordinate services with patients, providers, and payers to improve access and promote health in a cost-effective manner.
  6. Document patient data, interventions, and outcomes according to department guidelines.
  7. Stay informed on current trends and developments in the field through literature, conferences, and seminars.
  8. Monitor and improve processes and services for quality, cost-effectiveness, and efficiency, engaging in quality improvement activities.
Qualifications
Required
  • Associate's Degree in nursing, social work, occupational or physical therapy, or related health field
  • 2 years of relevant experience
  • Licensure as an RN or applicable license in Michigan, or other listed health professions with upon-hire certification
  • Basic Life Support (BLS) certification
  • Valid Michigan driver’s license
Preferred
  • Bachelor’s Degree in a related field
