Job summary

Piedmont Augusta Hospital is seeking a Social Worker to promote patient and family wellness and to develop transitional care plans for patients with complex psychosocial needs in inpatient and emergency department environments.

The role supports discharge planning and coordination across hospital and community resources, with a schedule of at least two days per week (business hours) and potential for a third day. LMSW licensure in Georgia is preferred.

Qualifications

• Master's Degree from an accredited social work program is required.
• No experience required; 2 years of experience in an acute or post-acute setting preferred.
• LMSW licensure in Georgia preferred.

Responsibilities

• Promote patient and family wellness and improved care outcomes.
• Manage, support and develop comprehensive transitional care plans for patients with complex psychosocial needs in Inpatient and Emergency Department environments.
• Provide care in inpatient and emergency department settings to promote seamless transitions.
