Job summary

Qualifications

• Master's Degree in Social Work.
• Licensure: LSWAIC, LMHC, LMFT, LASW, LICSW as required by department.
• Experience in hospital/acute setting or equivalent social service setting.

Responsibilities

• Administer assessment tools and gather data for assessment and service needs.
• Interview families to determine social work needs of the participant.
• Contact community resources to coordinate services.
• Provide initial and ongoing assessment of participant's social, emotional and cognitive status and needs, involving participant and staff.
• Ensure accurate documentation for all assessments, treatment plans and evaluations.
