Job summary

RWJBarnabas Health is seeking a Social Worker MSW at RWJUH Somerset in the Eating Disorders Unit. This salaried, day-shift role provides direct case management and operates within an interdisciplinary team to support patients and families.

Responsibilities include assessments, discharge planning, care transitions, and coordination with physicians and social work colleagues. NJ license and bilingual English/Spanish are preferred.

Qualifications

• MSW from an accredited School of Social Work.
• Active Social Work Licensure in New Jersey (LSW or LCSW).
• Flexibility to participate in weekend and holiday work rotation.
• Strongly prefer previous medical social work or hospital case management experience.
• Bilingual language skills in English and Spanish are highly preferred.
• Eligibility or interest in providing field instruction for graduate or undergraduate social work students.

Responsibilities

• Conduct comprehensive assessments and document in the EHR.
• Develop and implement individualized clinical interventions to support safe discharge and coordinate care.
• Coordinate discharge planning and transitions of care.
• Participate in interdisciplinary rounds (IDR) and communicate discharge planning updates.
• Provide crisis intervention and psychotherapeutic support addressing psychosocial distress.
• Utilize SDOH screenings to identify at-risk patients and coordinate referrals.
• Collaborate with physicians, nurses, and the care team to improve LOS and throughput.
• Ensure CMS Conditions of Participation and HIPAA compliance.
